REFERRAL PROCESS
Individuals are referred by social service agencies, educational institutions, employers, insurance companies, health care professionals, Ontario Works (OW), Workplace Safety & Insurance Board (WSIB), and other community service providers. Self-referrals or referrals made by family members are also accepted.
Referrals should be made in writing and mailed to:

Upon receipt of a referral, the participant will be contacted by phone or mail in order to set up an intake interview to determine:

the individual's current medical status |
|
limitations and goals |
|
obtain any necessary consents |
|
assess rehabilitation potential and motivation |
A participant's schedule and program will be developed, depending on his/her individual needs. An individual's understanding and acceptance of their roles and responsibilities in the rehabilitation process will also be discussed.
